THE COMPANY OFFERS TWO CHOICES FOR SUPPLEMENTAL COVERAGE UNDER THE MEDICAL PLAN.

BASIC – provided at no cost to you.

PROFESSIONAL SERVICES

Physiotherapist, Chiropractor, Osteopath, Naturopath, Podiatrist, Chiropodist, Registered Massage Therapist (medical referral required), Speech Therapist or Psychologist, up to a maximum per practitioner of:

• $400 per calendar year for Basic Plan.

• $750 per calendar year for Enhanced Plan.

VISION

Your Vision Benefit carries a maximum as follows:

• Prescription eye glasses and/or contact lenses or laser eye surgery, or medically necessary contact lenses

• $250 every 24 months (every 12 months for covered persons 17 years of age and under) for Basic Plan, and

• $325 every 24 months (every 12 months for covered persons 17 years of age and under) for Enhanced Plan.

• Visual training: $200 lifetime maximum provided they are dispensed by an Optometrist, an Optician or an Ophthalmologist.

AUDIO

Reimbursement will be made for standard hearing aids, repairs or replacement parts up to a maximum of:

• $500 every 3 years for Basic Plan.

• $600 every 3 years for Enhanced Plan.

Batteries are not eligible.

ADDITIONAL HIGHLIGHTS (BASIC & ENHANCED)

EYE EXAMINATIONS

Eye examinations performed by a registered, licensed Optometrist or

Physician are limited to one exam every 12 months for covered persons

17 years of age and under and every 24 months for covered persons 18

years of age and older up to a maximum of $70 (available only where eye

examinations are not covered by the provincial health insurance plan).

EMERGENCY TRANSPORTATION

Ambulance transportation, for land or air ambulance to the nearest

hospital equipped to provide the required treatment.

MEDICAL ITEMS

Prosthetic Appliances and Durable Medical Equipment as well as

replacements, repairs, fittings and adjustments of such devices. Contact

the Customer Service Centre to verify eligibility of a particular benefit.

 

PRIVATE DUTY NURSING IN THE HOME

Private duty nursing benefits carry a maximum of $25,000 per calendar

year for the services of a Registered Nurse (R.N.) or Registered Practical

Nurse (R.P.N.)/Licensed Practical Nurse (L.P.N.) in the home on a full or

part shift basis.

TRAVEL

• Travel benefits are eligible for the first 30 days per trip.

• Your maximum is $1,000,000 per covered person per calendar year

for Emergency Services; and $50,000 per calendar year for Referral

Services.

• Hospital and medical services are eligible only if your provincial health

insurance plan provides payment toward the cost of incurred services.

• Deductible and co-payments do not apply to Travel.

 LIMITS:

• Fertility drugs, $10,000 per lifetime.

• Smoking cessation program, one course of treatment in any 12 month period.

• Nicotine replacement products (patches, gum, lozenges, inhalers), $300 per lifetime.

• Vaccines, $1,000 per calendar year.

Quebec residents only: Legislation states that GSC is obligated to follow RAMQ reimbursement guidelines for all residents of Quebec. For those 65 years of age and under, GSC is primary payer.
